118 PHARMACY PRIVACY NOTICE

We process your personal data, which includes your name, contact details, prescription medicines and data from other pharmacy and health care services we provide to you for the purposes of: 

Your care – providing pharmacy services and care to you and, as appropriate, sharing your information with your GP and others in the wider NHS;

Our payments – sharing your information with the NHS Business Services Authority, others in the wider NHS, and sometimes Local Authorities, and only limited information to those external to the NHS who negotiate and check the accuracy of our payments; and,

Management– sharing only limited information with the NHS Business Services Authority and others in the wider NHS, and sometimes Local Authorities; as well as those external to the NHS who ensure we maintain appropriate professional and service standards and that your declarations and ours are accurate.  

We hold your information for as long as advised by the NHS. You have a right to a copy of the information we hold about you, generally without charge. You may seek to correct any inaccurate information. 

We process your personal data in the performance of a task in the public interest, for the provision of healthcare and treatment and the management of healthcare systems. A pharmacist is responsible for the confidentiality of your information. You may object to us holding your information. You may also lodge a complaint with the Information Commissioner's Office, Wycliffe House, Water Lane, Wilmslow, Cheshire SK9 5AF. Please ask if you want more information.

FREEDOM OF INFORMATION

As a provider of services to the National Health Service, 118 Pharmacy Ltd is a public authority for the purposes of the Freedom of Information Act 2000 and the Freedom of Information (Scotland) Act 2002. These Acts impose on us a legal obligation to provide information to anybody making a request through an approved publication scheme (in England this is also known as a Guide to Information). The general presumption is that information relating to our public services must be disclosed. However, there are a number of exemptions set out in the relevant Act which enable us to limit what we provide.

THE INFORMATION WE COLLECT

In order to provide you with the services you require, we will collect personal information about you from our website, telephone conversations, emails and written and verbal communications. These will include, for example, your name, address, email address, phone number and details that are relevant to the service(s) in which you are interested.

We may also collect sensitive personal data concerning health matters from or about you if you our pharmacy service.

We may supplement the information that you provide with other information that we obtain from our dealings with you or which we receive from other organisations, such as other companies in our group.

If you provide information to us about any person other than yourself, you must ensure that they understand how their information will be used and that you are authorised to disclose it to us, and to consent to its use on their behalf, before doing so.

USE OF YOUR INFORMATION OUTSIDE THE EUROPEAN UNION

Some of the organisations to which we may disclose personal information may be situated outside of the European Union. In order to provide the products and services you require, we may need to transfer your/their personal information to these countries some of which do not have laws that protect privacy rights as extensively as in the United Kingdom. You have the right to receive a copy of the personal information that we hold about you. The same right applies to any other person whose personal information you provide to us. We may charge a small fee towards the cost of administering any request made. The current fee is £15.00.
